Clinical Care Nurse RN
Virginia (Remote)
$34.50/HR
Monday–Friday, 8:00 AM – 5:00 PM
The Clinical Care Nurse (RN) position is a remote, clinic-oriented role aimed at enhancing patient outcomes via quality improvement, Transitions of Care (TOC), and population health strategies. This role is pivotal in minimizing unnecessary hospital use, aiding in post-discharge processes, and enhancing Medicare Advantage Stars and HEDIS performance. The RN will utilize patient outreach and educational efforts alongside data-centric approaches to pinpoint care discrepancies, engage with patients and providers, and apply evidence-informed practices that boost overall health results.
Key Responsibilities of Clinical Care RN:
- Manage Transitions of Care, ensuring effective post-discharge follow-up and preventing readmissions.
- Evaluate clinical data (including Athena EMR, DataHub) to detect care discrepancies and support HEDIS/quality objectives.
- Engage in patient outreach through phone or telehealth to educate on chronic illnesses, medication management, and preventive health measures.
- Conduct medication reconciliation and verify patient comprehension post-discharge.
- Work in collaboration with interdisciplinary teams to enhance workflows and patient results.
- Aid in population health programs, particularly regarding chronic illnesses such as diabetes and hypertension.
- Accurately record all patient interactions, ensuring compliance with CMS and HIPAA regulations.
- Identify obstacles to care and partake in quality improvement initiatives.
Requirements for Clinical Care RN:
- Bachelor’s degree in Nursing.
- Current, unrestricted RN license (must be active in the required state).
- At least 3+ years of clinical nursing experience (TOC, quality, managed care, or population health experience is preferred).
- Familiarity with Medicare Advantage Stars, HEDIS, or value-based care is advantageous.
- Skilled in using EMRs (Athena preferred) and Microsoft Office.
- Excellent communication, critical thinking, and patient engagement abilities.
- Capable of working autonomously in a remote environment.
- Bilingual capabilities (English/Spanish or Creole) are a bonus.
- Must submit a valid RN license and a comprehensive NURSYS report upon application.
